The Physical Therapy Practice Act in the State of California requires us to have a physician's diagnosis of your condition in order for us to initiate physical therapy treatment. Please bring your physician referral to your first appointment. Most insurance plans require a physician referral to cover physical therapy services.

We are in network providers for Medicare, Blue Shield, Anthem Blue Cross, United Healthcare, and Tricare. We are out of network providers for Aetna, Cigna, HealthNet and all other PPO plans. We are not providers for HMO plans.

We offer a 20% discount for cash-paying clientele.

We request notification of cancellation of an appointment by 10 am the day before your scheduled appointment. A $65 cancellation fee for late cancellations and no-shows will be charged.

We are happy to assist you in determining your level of coverage for outpatient physical therapy services. We verify coverage before initiating physical therapy. Ultimately, it is your responsibility to know your health plan benefits for outpatient physical therapy. Please contact your carrier directly for the details of your plan.
