Insurance: A physician’s diagnosis of your condition is required before beginning physical therapy treatment. Please bring your physician referral to your first appointment. Most insurance plans require a physician referral to cover physical therapy services. We are providers for Medicare, Blue Shield, United Healthcare, and many other PPO plans. We are out of network providers for some PPO’s, such as Blue Cross and Aetna. We are not providers for HMO plans. It is your responsibility to know your health plan benefits for outpatient physical therapy. Please contact your carrier directly for the details of your plan. We would be happy to assist you in determining your level of coverage. Worker’s Compensation claims require authorization for physical therapy prior to starting treatment. (Some other insurance plans may also require pre-authorization.) Please have your physician’s office send us a referral along with your billing information and we will request treatment authorization for you.
